REQUEST A WIN/LOSS STATEMENT

To request a statement of your wins and losses for an entire year, simply complete the form, selecting the year you'd like reported. The information provided in the Win/Loss Statement is only for the amount of gaming play reflected by the use of your Player's Club card.

***NOTE The name, address, and card information you provide will be used to verify your identity. We will only mail the information to the address we have on record for you. Providing your phone number will help us clear up any discrepencies.
